  • School Nurse The following are requirements for all Kindergarten students, whether your child is attending in-person instruction or receiving instruction virtually. All students are required to meet health examination and immunization requirements. Students not in compliance will be subject to exclusion from school per the Illinois state guidelines.

    1. HEALTH EXAMINATION AND VISION FORMS ARE due by October 15, 2020

    • must include lead screening for children who are six years or younger and diabetes screening
    • must be dated within one year prior to the first day of school (August 19, 2020)

    Certificate of Child Health Examination-English

    REQUIRED IMMUNIZATIONS

    • DTP/DTaP – 4 or more doses with last booster after 4th birthday
    • Polio – 4 or more doses with last booster after 4th birthday
    • MMR – 2 doses with first dose on or after 1st birthday
    • Varicella (Chicken Pox) vaccine-2 doses with first dose on or after 1st birthday and 2nd dose no less than four weeks after the first dose, or proof of varicella disease.

    Parents requesting religious or medical exemptions to any school health requirements must provide the appropriate documentation prior to the due date.     2. VISION EXAMINATION due by October 15, 2020

    3. DENTAL EXAMINATION due by May 15, 2021.

    The dental examination must have taken place within 18 months prior to May 15 of the school year. The IDPH Dental Exam Form and Dental Exam Waiver form can be found here.

     

    The following are requirements for all 6th students, whether your child is attending in-person instruction or receiving instruction virtually. All students are required to meet health examination and immunization requirements. Students not in compliance will be subject to exclusion from school per the Illinois state guidelines.

    1. HEALTH EXAMINATION due by October 15, 2020.

    Physical examination must have taken place within one year prior to school start date (August 19, 2020).

    A student not meeting this requirement is subject to being excluded from school. 

    Certificate of Child Health Examination-English

    REQUIRED IMMUNIZATIONS:

    • Tdap - 1 dose before 6th grade year
    • Meningococcal conjugate vaccination - 1 dose received on or after 11 years of age

    PROOF OF PREVIOUS REQUIREMENTS:

    • DTP/DTaP – 4 or more doses with last booster after 4th birthday
    • Polio – 4 or more doses with last booster after 4th birthday
    • MMR – 2 doses with first dose on or after 1st birthday
    • Hepatitis B vaccine - 3 doses
    • Varicella (Chicken Pox) vaccine - 2 doses with first dose on or after 1st birthday and 2nd dose no less than four weeks after the first dose, or proof of varicella disease

    2. DENTAL EXAMINATION due by May 15, 2021.

    The dental examination must have taken place no earlier than 18 months prior to May 15, 2021.

    All students are required to meet health examination and immunization requirements. Students not in compliance will be subject to exclusion from school.
