A dental exam is required for all students in kindergarten, second and sixth grade.  Please submit one to the health office by May15, 2023.   Dental exams dated between 11/15/21-5/30/23 are valid for that grade.

An eye exam is required for all students new to Illinois and all kindergarten students.  If you have not submitted one, please turn one in as soon as possible.  Eye exams dated between 8/16/21-5/15/23 are valid for the 22/23 school year.

Parents may submit physicals, eye exams and dental exam forms to the health office now for 2023/24 school year.

Students who do not have a physical exam and current immunizations by 10/15/23 will be excluded from school until they submit these forms.
