Registration/Enrollment 2024-25
-
Online Registration for 2024-25 opens January 30, 2024
and closes July 1, 2024!Every student — new and returning — must register each year.
It is very important for families to register or re-register as early as possible. The district uses our enrollment numbers to determine staffing at each of our schools.
Below are the steps to enroll your child in the 2024-25 school year in District 15. All families can register starting on January 30, 2024, even if you haven't heard a final decision on your child’s placement. Should your student be placed in a program at a school other than your home school, adjustments will be made to your registration accordingly.
1. Determine Eligibility
-
Do you live in District 15's boundaries?
Per Illinois law and Board of Education policy, only students who are legal residents of District 15 may attend school in the district. Generally, a student’s residency (proof of living situation) is the same as the person who has legal custody of the student. The person registering the student must submit proof of legal custody and residency (proof of living situation).
Proof of residency (proof of living situation) is required for all NEW students (kindergarten and students new to the district), families flagged by the third-party verification system, and all families with month-to-month and yearly LEASES.
Visit the Residency (Proof of Living Situation) page to determine eligibility.
Use the Map My School tool* to locate your school on the updated boundary map for 2024-25. (Enter the student's home address and click the search button to get assigned schools, or click on the map to get assigned schools at any location.) Please keep in mind that the Map My Schools tool may not reflect school assignments due to programmatic placement. The Map My School tool shows a student’s home school based on their address.
** All efforts are made to ensure the accuracy of this tool. The school district reserves the right to make final school assignment determinations.
-
Age - Is your child 5 years or older (on or before September 1)?
To begin kindergarten in Illinois, a child should be 5 years old on or before Sept. 1 of the kindergarten year and live within the boundaries of the school district.
If your child will not be 5 on or before September 1, District 15 does offer early childhood and pre-school options.
2. Complete Online Registration by July 1
-
** REGISTRATION LINKS **
For families that are NEW to District 15: use the ONLINE REGISTRATION KIOSK to start a new registration application to enroll your child in a District 15 school.
For families who have children already enrolled in District 15: use the CAMPUS PARENT link to register your child(ren) for the upcoming school year, including any children new to the District. You will need your parent username and password to sign in to your Campus Parent account. If you forgot your password or username, click on the "Forgot" links.
See the tutorials below that will walk you through the registration process (available in English, Spanish, and Ukrainian). If you need translation assistance to complete online registration, please contact your child's school. An interpreter will be provided.
To submit a question regarding online registration, please fill out our online form. A District 15 representative will get back to you quickly.
Have the following information ready as you complete the application:
- Proof of residency (living situation) documents - Proof of residency (living situation) is required for all incoming kindergarten students, all families with leasing agreements, and families with students new to the District. You have the ability to upload required Proof of Residency (living situation) documents in Infinite Campus. If you are unable to upload these documents, you will need to bring them in person to the school office to provide your Proof of Residency(living situation).
- Household information (address and phone numbers)
- Parent/guardian information (work and cell phone numbers, email addresses)
- Student emergency contacts (names, home phone, cell phones, work phones, emails)
- Student's birth certificate (you'll need to enter their full legal name as it appears on their birth certificate)
- Student's prior school attendance information, if applicable (school name, city, state, country, phone)
- Name of primary health and primary dental providers (names, phone numbers)
- Student health history - Students entering CCSD15 for the first time are required to submit a State of IL Certificate of Child Health Examination form with parent/guardian and doctor signatures prior to attending school. There are a number of questions on the online registration form regarding your child's health history and medical conditions, so having this form in hand may be helpful. Student health history will need to be re-entered each year, it does not roll over from previous years.
Note: if you are unable to complete the entire application at one time, don't worry, you can come back to it later to complete it. Remember to write down your application number, you will need this to return to the application. You will be able to print or save a copy of the application for your records before submitting the application. Once the application is SUBMITTED, you will no longer be able to make changes. Please contact your school for any necessary corrections. If needed, you will receive an email from the school letting you know what documents/information will be needed to complete your registration.

3. Provide Required Documentation to the School
-
Birth Certificate or Passport
All students who are newly registering with the district are required to provide a Birth Certificate or Passport (original with seal). Official birth certificates are issued by the county clerk in the county in which the child was born. Hospital certificates are not acceptable. A certified copy of the child’s birth certificate must be provided within 30 days of enrollment.
Please bring a certified copy of your child’s birth certificate (with seal) or other government document verifying date of birth. An official certificate of every birth should be on file in the locality where the event occurred. These records are filed permanently in a State vital statistics office or in a city, county, or other local office.
To obtain a certified copy of a birth certificate, write or go to the vital statistics office in the State or area where the event occurred, or visit the National Center for Health Statistics (www.cdc.gov/nchs/w2w/index.htm) for information on where to obtain an official birth certificate.
-
Home Language Survey
A Home Language Survey is administered to all students newly registered in the district to help us determine if your child is eligible for language support services. Families will be provided with the Home Language Survey in their spoken language.

Residency (proof of living situation) in District 15 is required for all incoming Early Childhood students, Kindergarten students, students new to the District, existing families with a lease, and families that are flagged by our third-party residency (living situation) verification system to complete the registration process.
District 15 requires all families with NEW students (kindergarten and students new to the district), families flagged by the third-party verification system, and all families with month-to-month and yearly LEASES to prove residency each year.
For the 2024-25 school year, in the case of a lease, please note that if the copy of the lease provided expires before September 6, 2024, the family will need to provide an UPDATED lease prior to the start of the school year to remain enrolled for the 2024-2025 school year. The lease needs to show that the family lives in District 15's boundaries at the start of the 2024-2025 school year.

Health Plan - Individualized Health Care Plans
An increased awareness of asthma and allergies is needed to keep students safe at school. The Health Services staff maintains a list of students with asthma to share with staff on a need-to-know basis. Asthma information and recommended actions for responding to emergency events, including emergency medication, are provided to staff. Illinois law requires students with asthma to provide an Asthma Action Plan that guides staff response.
A Diabetes Care Plan will be developed and implemented for students diagnosed with diabetes, per the Care of Students with Diabetes Act. When parents/guardians submit a medical Diabetes Medical Management Plan to the school district, the plan will then be incorporated into an IEP or Section 504 plan, by a certified school nurse with protocol for implementation and staff training.
Food Allergy and Anaphylaxis Emergency Care Plan
If your child has life-threatening anaphylactic allergies (including food allergies), please notify your school nurse and print the Food Allergy and Anaphylaxis Emergency Care Plan for completion by your physician, and submit it to your school's health nurse. The nurse maintains a list of students who have anaphylactic allergies to share with staff on a need-to-know basis and can assist you with the necessary documents needed for school.
For students with epilepsy, a Seizure Action Plan is needed to provide appropriate care while the student is at school. All school employees receive training in the basics of seizure recognition and first aid and appropriate emergency protocols.
-
Health Plan - Medication Authorization Forms
CCSD15 believes that prescription and nonprescription medication should be administered in the home. However, if it is in the best educational and health interest of the child to take prescribed medication during the school day, then a Medication Authorization Form must be completed. This pertains to all medications (prescription, over the counter, and emergency). The Medication Authorization Form is available online at www.ccsd15.net/MedicationAdministration or in the school nurse's office. Medication must be brought to the school by the parent/guardian in the original container.
-
Transportation - Download the Stopfinder App - Bus Tracking system
The Stopfinder Parent App is the new bus GPS system which can be used by families to help track their child's bus. It is designed by parents for parents/guardians to keep them in the know - in real-time - where their child's bus is and when their student arrives at school. To use it, parents/guardians need to download the Stopfinder mobile app from the App Store or get it on Google Play.
An invitation to join Stopfinder is sent in August to the PRIMARY parent/guardian email address listed for the student in Infinite Campus (only the primary parent/guardian is sent the invitation). That link expires after 90 days. After you confirm the registration email sent to you, click the link and follow the instructions to become a subscriber. Once the subscription has been activated, the primary parent/guardian contact can share the link to register with other individuals/caregivers.
-
Transportation - Paid Busing
In most cases, students who live more than 1.5 miles from their home school will be provided free bus service through CCSD15. State funding is not provided for students living within 1.5 miles of their schools. These students may ride the bus for a fee determined annually by the Board of Education provided that space is available and a route already exists. (No stops will be made less than one-half mile from the school building.)
An Application for Paid Transportation will be available on the Transportation webpage for download on/after September 5, 2024. Please submit a separate application for each student for whom you choose paid transportation.
Due to the bus driver shortage, paid ridership continues to be limited and only offered if space is available on an existing bus route. Applications will be accepted, but payment will not be accepted until a parent receives notification that their child has been assigned to a bus route. Bus availability will be evaluated after October 1.
Paid Rider Transportation Fees for 2024-25 are:
- One child - $370
- Two children - $470
- Three or more children - $520
-
Transportation - Voluntary Opt Out of Bus Service
In an effort to reduce bus route times, we are asking parents/guardians who are eligible for free bus service, but do not need or intend to use the bus service, or only need transportation one way, to voluntarily opt out. If a student lives 1.5 miles or more away from school, students are entitled to free bus transportation. Unless a family notifies us that they do not want transportation, their children are placed on a bus route. Some families do not want to use transportation and choose to transport their children to and from school themselves. If we remove students from bus routes that are NOT going to use district transportation, we can reduce bus route times by eliminating the bus stopping at that assigned bus stop. At present, the bus stops at all assigned stops on the route and waits 3 minutes for the students to board the bus. If there are no students at that stop, the bus still waits for 3 minutes before moving onto the next stop.
If you DO NOT want your child or DO NOT need your child to use district transportation, or only need transportation one way (to or from school), please fill out this google form so we can remove your child(ren) from the route(s). Please fill out the form and indicate which route(s) you do NOT need transportation for.
If your child(ren) is assigned to a bus route and you wish to continue using district transportation, you do not need to do anything. The OPT OUT form is ONLY for families that do not need D15 bus transportation.

Parent/Student Handbook
The Parent/Student Handbook offers information pertinent to the District as a whole. Some programs and procedures may differ from school to school, based on the size and location of the building, the grades served, the facilities available, and the programs conducted at the school. Your school administrators will inform you of these additional procedures. The handbook is intended to be a handy resource, but is not meant to take the place of personal communication between home and school. Please feel free to call your child’s school if you have any questions on programs or procedures, or consult your school’s website. Please note, the Parent/Student Handbook for 2024-25 will be updated and posted before the start of the school year.

What is my "home school"?
A "home school" is the family's school assignment based on their home address.
The Map My School tool allows families to type in their address to see where their house is on the updated boundary map, showing their "home school" assignment.
*Please keep in mind that the Map My Schools tool may not reflect school assignments due to programmatic placement. The Map My School tool shows a student’s home school assignment based on their address.
-
Which school will my child attend?
Students are usually assigned to their neighborhood school, but in special circumstances, they may be assigned to schools outside their neighborhood.
Map My School Tool
District 15 is excited to share its new Map My School tool which allows families to type in their address to see where their house is on the updated boundary map. *Please keep in mind that the Map My Schools tool may not reflect school assignments due to programmatic placement. The Map My School tool shows a student’s home school assignment based on their address.

What if I want to stay at my current school next year?
The District has an Intra-District Transfer process in which parents can apply to have their child attend a school other than their home school. The form will be posted on the website after spring break. Families requesting a transfer because it is their child's last year in a school are given preference. We do need to look at the enrollment at a school and grade level to determine if we can grant the request. If an Intra-District Transfer request is granted, the family will need to provide their own transportation to and from school as our buses are routed to residences in the home school attendance area for each school.
-
If I move over the summer, where will I go to school next year?
If you move over the summer, your Home School may change based on your new address. If your new address will still be in School District 15 boundaries, fill out the online registration now. Your registration will still be applicable, but you will need to provide a new proof of residency (living situation) after moving. You can use the Map My School tool to see where your new address is on the updated boundary map.
-
My address is not correct in Infinite Campus, can you update our Home School?
We pull addresses based on the information uploaded to Infinite Campus. If you need to update that information, please contact your school. If you register now and your school is updated for the next school year, we can take care of that switch in our system on the back end.
-
What if one parent is moving, can the student stay in their current school?
A student’s home school is based on who they live with 51% of the time.
-
How can I enroll if I have a temporary living situation or am homeless?
In the event that a family is unable to provide any needed documentation because of a temporary living situation, or if you believe you're eligible for services under the McKinney-Vento Homeless Assistance Act, please contact the District 15 liaison for homeless children:

Current K-4 Dual Language students enrolled in the Dual Language program will continue on at the school they currently attend. My child attends a local daycare before and/or after school. Is there transportation available to/from a day care?
Only children who are residents of District 15 may attend District 15 schools from a daycare center from within the district. The daycare must be within the home school boundaries of the student. Please review the list of local daycare centers and if School District 15 provides transportation from the daycare to the school, or if the daycare center provides transportation to the school.
- If your child’s daycare center is located within the school attendance boundary associated with your home address, your child may be eligible for district-provided transportation if the daycare center is located 1.5 or more miles away from the student’s home school or if hazardous crossings exist as defined by the State of Illinois.
- If your child’s daycare center is not located within the school attendance boundary associated with your home address, and you would like the District to consider allowing your child to attend another school where the daycare center is located, you will need to request an Intra-District Transfer Request on behalf of your child.
The Intra-District Transfer Committee makes a final determination on the status of each transfer request received. In the event that the Intra-District Transfer Committee approves an intra-district transfer request, parents are responsible for all costs associated with providing transportation for their child to and from school. Requests are approved on a year-to-year basis.
PLEASE NOTE: Space for pay riders is limited and only offered if space permits for 2024-25.
- If your child’s daycare center is located within the school attendance boundary associated with your home address, your child may be eligible for district-provided transportation if the daycare center is located 1.5 or more miles away from the student’s home school or if hazardous crossings exist as defined by the State of Illinois.
-
Can I request that my child attend a different school in the district?
Under special circumstances, and on a very limited basis, parents/guardians may request an Intra-District Transfer of their child(ren) from their assigned home school to another intra-district school. Requests will only be considered upon completing the requested application and providing supporting documentation within the announced timeline. Requests are handled by the District 15 Intra-District Transfer Committee. Each spring, District 15 will announce the dates and deadline to submit requests for consideration for the upcoming school year.
