Intramural & Club Sports

In order to participate in an intramural or club sport at the elementary or junior high level, all students must have the following on file:
- A current physical on file with the school nurse. Physicals are valid for 13 months.
- Emergency Medical Authorization Card
- Concussion Information Form
- A signed CCSD15 Code of Conduct Contract
Students participating in these activities are still expected to adhere to the same Sportsmanship Expectations and CCSD15 Code of Conduct Contract as interscholastic sports participants.

COVID-19 Update
In conformance with mandated health and safety guidelines, District 15 may modify or cancel intramural, club and interscholastic sport offerings for all or part of 2020-21, and will adhere to Illinois Elementary School Association (IESA) decisions regarding the operations of interscholastic sports for 2020-21.
