Illinois Youth Survey (IYS)

  • In February 2022, all District 15 8th-grade students will be asked to participate in the statewide Illinois Youth Survey (IYS).  

    The IYS is used to gather information to help plan important prevention and intervention programs to reduce youth drug use and associated risk behaviors. The IYS will be taken on the Chromebook and takes about 25-40 minutes to complete.

    The IYS is entirely anonymous. Students do not put their names or any other identifying information on the survey booklet. Our schools will receive a report of overall results, but we and the sponsoring agencies will not know which students gave which responses. All results from the survey will be presented in a group summary format only, like many opinion polls.

    This survey is voluntary, so your child will participate only if you and your child agree to do so. Please read the Parent Notification Letter and return the Parent Opt-Out Form (ENGLISH | SPANISH) to your child's school if you would like to exclude your child from participation. (You do not have to complete this form if you are allowing your child to participate.) Students not taking the survey will have an appropriate alternative activity during the survey time. There is no penalty for you or your child for not participating in the survey. Please contact your school if you would like to discuss the survey or have any additional questions.
