Under special circumstances, and on a very limited basis, parents may request an Intra-District Transfer of their child(ren) from their assigned home school to another intra-district school. Requests will only be considered upon completing the requested application and providing supporting documentation within the announced timeline. Requests are handled by the District 15 Intra-District Transfer Committee. Each spring, District 15 will announce the dates and deadline to submit requests for consideration for the upcoming school year. 

2024-25 School Year

**The 2024-25 Intra-District Transfer Request form will be available on April 2, 2024.**
Requests will be accepted from May 1 - June 15, 2024.

Intra-district transfer requests will be reviewed according to these procedures:

Please use the D15 Intra-District Transfer Request Form to submit a request:

1. Student Information

Student(s) must be registered in District 15 in order to apply. Parents may list up to four (4) children from the same family and home address per request form. One request per student per year.

2. Transfer Request

Include the name of the transfer from school (assigned school) and the name of the school you wish to attend (receiving school).

3. Reason for Transfer

Check one reason for transfer for each student listed. See the form for details. Be sure to include any accompanying documentation from certified medical or mental health personnel and school administration from the home school, and/or Parent Affidavits and Daycare Affidavits, as required.

4. Terms of Agreement

I understand and accept the below terms associated with the intra-district transfer request that I have made on behalf of my child:

• I understand, in the event that my request is approved, paid transportation by the District will not be available, and that I am responsible for transporting my child to and from school.
• UNTIL notified by the D15 Intra-District Transfer Committee, this request is NOT approved and I must register my child in the home attendance area school.
• I will provide proof of residency in the assigned home school.
• Due to enrollment changes that take place throughout the year and into the summer months, I will more than likely not be notified of the D15 Intra-District Transfer Committee’s decision until a week or two before the start of the school year.
• My request will more than likely not be approved if doing so will exceed class-size targets established by either the state of Illinois and/or the District 15 Board of Education.
• My request will not be approved in the event that it will require the district to hire additional staff.
• If this transfer request is approved, it is only valid for one (1) school year and that I will need to make a similar request each year thereafter.
• An approved transfer may be rescinded and future requests not approved by the D15 Intra-District Transfer Committee in the event that:
  • My child engages in misconduct that is disruptive to the educational environment.
  • My child demonstrates a pattern of unexcused tardiness or absences.
  • My child does not maintain good academic standing.
  • My conduct toward staff, administration, other parents, and students is not respectful, appropriate or is disruptive pursuant to Board of Education Policy 8:30, Visitors to and Conduct on District Property.
  • I elect not to respond to or meaningfully participate in required meetings that staff and/or administration have invited me to attend on behalf of my child.
  • Other relevant reasons exist as determined by the D15 Intra-District Transfer Committee.
    
    

5. Be sure to sign the D15 Intra-District Transfer Request form and return all necessary documents/attachments via:

Bajo ciertas circunstancias especiales, los Padres pueden solicitar que su(s) hijo(s), sean transferidos de la escuela que les corresponde a otra, también, dentro del distrito. Las solicitudes sólo se considerarán después de completar la solicitud solicitada y proporcionar la documentación de respaldo dentro del plazo anunciado. Las solicitudes son manejadas por el Comité de Transferencias Intra-Distrito del Distrito 15. Año Escolar 2024-25

Las solicitudes de traslado dentro del distrito se examinarán de acuerdo con estos procedimientos.

Utilice el formulario de solicitud de traslado dentro del distrito D15 para presentar una solicitud:

1. Información del estudiante

Los estudiantes deben estar registrados en el Distrito 15 para poder solicitarlo. Los padres pueden listar hasta cuatro (4) niños de la misma familia y domicilio por formulario. Una solicitud por estudiante por año.

2. Solicitud de transferencia

Incluya el nombre de la escuela de transferencia (escuela asignada) y el nombre de la escuela a la que desea asistir (escuela receptora).

3. Motivo de la transferencia

Marque una razón para la transferencia por cada estudiante indicado. Vea el formulario para más detalles. Asegúrese de incluir cualquier documentación adjunta del personal médico o de salud mental certificado y de la administración escolar de la escuela de origen, y/o declaraciones juradas de los padres y de la guardería, según se requiera. 

4. Términos del Acuerdo

Entiendo y acepto los siguientes términos asociados con la solicitud de transferencia dentro del distrito que he hecho en nombre de mi hijo/a:

• Entiendo que, en el caso de que mi solicitud sea aprobada, el transporte pagado por el Distrito no estará disponible, y que soy responsable de transportar a mi hijo/a hacia y desde la escuela.
• HASTA QUE SEA NOTIFICADO por el Comité de Transferencia dentro del Distrito del D15, esta solicitud NO es aprobada y debo inscribir a mi hijo/a en la escuela del área de asistencia de su hogar.
• Proporcionaré una prueba de residencia en la escuela de origen asignada.
• Debido a los cambios de matrícula que tienen lugar a lo largo del año y en los meses de verano, es más que probable que no se me notifique la decisión del Comité de Transferencia dentro del Distrito del D15 hasta una o dos semanas antes del comienzo del año escolar.
• Es más que probable que mi solicitud no sea aprobada si al hacerlo se exceden los objetivos de tamaño de clase establecidos por el estado de Illinois y/o la Junta de Educación del Distrito 15.
• Mi solicitud no será aprobada en caso de que requiera que el distrito contrate personal adicional.
• Si esta solicitud de transferencia es aprobada, sólo es válida por un (1) año escolar y tendré que hacer una solicitud similar cada año a partir de entonces.
• Una transferencia aprobada puede ser anulada y las futuras solicitudes no aprobadas por el Comité de Transferencia dentro del Distrito del D15 en el caso de que:
  • Mi hijo/a tiene una mala conducta que perturba el entorno educativo.
  • Mi hijo/a demuestra un patrón de tardanzas o ausencias injustificadas.
  • Mi hijo/a no mantiene una buena posición académica.
  • Mi conducta hacia el personal, la administración, otros padres y estudiantes no es respetuosa, apropiada o es perturbadora de acuerdo con la Política 8:30 del Consejo de Educación, Visitantes y Conducta en la Propiedad del Distrito.
  • Elijo no responder o participar significativamente en las reuniones requeridas a las que el personal y/o la administración me han invitado a asistir en nombre de mi hijo/a.
  • Existen otras razones relevantes según lo determinado por el Comité de Transferencia dentro del Distrito D15.
    
    

5. Asegúrese de firmar el Formulario de Solicitud de Traslado dentro del Distrito 15 y envíe todos los documentos/adjuntos necesarios a través de:
