Enrollment Procedures: Kindergarten and New Student Registration

Kindergarten and New Student Registration Scheduled for Wednesday, February 15, 2012

 

Kindergarten and new student registration for students entering Community Consolidated School District 15 will be held Wednesday, February 15, 2012, from 9:30–11 a.m., 1–3 p.m., and 6–8 p.m. at all District 15 elementary schools.

 

For the 2012-13 school year, kindergarten students must be five years old on or before September 1, 2012.

 

Enrollment requires the following information:

 

  1. A transfer slip from the previous school attended (if applicable)
  2. An official birth certificate (not a hospital-issued certificate) or other government document verifying the date of birth
  3. Physician's telephone number and address, if known
  4. The name and phone number of two reliable local persons to call if you are unavailable in an emergency
  5. Two proofs of residency (copies of drivers license, lease, or utility bills)
  6. Any pertinent health information including health, dental, and eye exams at required intervals (see below for more information). A physical examination is valid so long as it was conducted on or after August 27, 2011.

 

Students are usually assigned to their neighborhood school, but in special circumstances, they may be assigned to schools outside their neighborhoods. Parents who are not sure which school their children will attend may call the district's Joseph M. Kiszka Educational Service Center at 847-963-3000.

The first day of the 2012-13 school year will be Monday, August 27, 2012, for Grades 1–8. Kindergarten and early childhood classes will begin Tuesday, August 28, 2012. School supply lists for each school are available from the schools and also are posted on each school's webpage.

 

Early Entrance to Kindergarten or First Grade

Required Health Examinations and Immunization Recording

 

For the purpose of safeguarding the health of children, District 15 enforces Illinois health requirements. These requirements consist of health examinations, immunizations, dental and eye examinations specific to grade and age.

 

Health exams are required for students in kindergarten, sixth grade, and those who are new to the school. The Certificate of Child Health Examination form is available here (English PDF) and in the school office.


The exam, completed and signed by a doctor, must be dated within one year prior to the first day of school and must include lead screening for children who are six years or younger and diabetes screening. A tuberculosis (TB) skin test is recommended. The parent/guardiancompletes and signs the Health History section of the form.

In addition, annual health examinations are required for interscholastic sports participation.

 

The required school immunizations are:

 

  • Diphtheria, tetanus and pertussis (DPT)
  • Diphtheria, tetanus and pertussis booster adolescent (Tdap)
  • Polio (IPV)
  • Measles, mumps and rubella (MMR)
  • Haemophilus influenzae (Hib)
  • Chicken pox/Varicella (Varivax)
  • Hepatitis (HBV)

 

On October 15, students will be excluded from school if the health exam and the required immunizations have not been given to the school. New students who register mid-term shall have 30 days following registration to comply.

 

Dental exams are required for students in kindergarten, second grade, and sixth grade. The Illinois Department of Public Health (IDPH) Dental Examination form is available here (English PDF/Spanish PDF) and in the school office. If a student fails to present proof of a dental exam by May 15, the school may hold the student's report card until the student presents proof: (1) of a completed dental exam, or (2) that a dental exam will take place within 60 days after May 15.

 

Eye exams are required for students entering kindergarten and those entering Illinois schools for the first time. The State of Illinois Eye Examination Report is available here (English PDF) and in the school office. If a student fails to present proof of an eye exam by October 15, the school may hold the student's report card until the student presents proof: (1) of a completed eye exam, or (2) that an eye exam will take place within 60 days after October 15.

 

Health Information Survey

 

A Health Information Survey [SS-401] (English Word/Spanish Word) form must be completed and turned in to the health office for each child prior to the start of school. This is needed in order to share pertinent health information with school personnel on an as needed basis. This requirement is due to the confidentiality of student health records as defined in HIPAA (Health Insurance Portability and Accountability Act of 1996) and FERPA (Family Educational Rights and Privacy Act). A Health Information Survey form is mailed to homes each summer and is available on the district's Web site (see above for links).

Change of Address, Telephone Number or Name

 

In case of emergency, it is very important that the school has the most current address, home, work, and cell phone numbers.

 

Please notify the school promptly of any change of address, home, work, or cell phone numbers, name, emergency information, or any otherinformation that might be pertinent to school records. The notification should be in writing.

 

Race and Ethnicity Reporting

 

In fall 2007, the U.S. Department of Education issued new guidance on the collection and reporting of race and ethnicity data for public school students and staff. The guidance implements new federal race and ethnicity categories that were developed to obtain a more accurate picture of the nation's diversity. The new data collection process requires respondents to answer a two-part question, indicating ethnicity first and then one or more of five races. (In the past, individuals were allowed to choose only one race or ethnicity category.)

 

The Illinois State Board of Education (ISBE) will use the new categories starting with data to be reported for the 2010-11 school year. This requires school districts to re-identify race and ethnicity for all students—and the identification is to be done by parents or guardians. If a student's parents or guardians decline to indicate race and/or ethnicity, observer identification by school district staff is required.

 

The new race and ethnicity data will be used in the same manner as previously collected data, e.g., in reporting and analyzing test results by race and ethnicity. The information will not be used to check immigration status, and the confidentiality of individual student information will be protected.
