Walk-in Registration set for July 31, 2008, at District 15 Schools
Walk-in registration for students who will be attending District 15 schools in the 2008-09 school year but are not yet registered will be held on Thursday, July 31, 2008, at all district schools. Hours are 7:30 a.m. to 1 p.m. and 4 to 7 p.m.
Parents who are not certain which school their child will attend may call the District 15 office at 847-963-3000. To register a student, parents must bring:
- a transfer slip from the previous school attended (if applicable);
- an official birth certificate (not a hospital-issued certificate) or other government document verifying the date of birth;
- physician's telephone number and address, if known;
- the name and phone number two of a reliable local persons to call if you are unavailable in an emergency;
- two proofs of residency (copies of driver's license, lease, or utility bills); and
- any pertinent health information including health, dental, and eye examinations at required intervals (see Enrollment Procedures for more information).
Enrollment information soon will be mailed to parents of students who attended District 15 schools during the 2007-08 school year. Re-registrations which have not yet been completed should be completed and returned immediately.
The first day of the 2008-09 school year will be Thursday, August 21, for grades one through eight. Kindergarten and early childhood classes will begin Friday, August 22. School supply lists for each school are available from the schools and also are posted on the District 15 Web site at http://www.ccsd15.net/OurSchools/.
