Kindergarten Registration Set for April 18, 2006
Kindergarten registration for students entering Community Consolidated School District 15 will be held Tuesday, April 18, 2006, at all elementary schools in the district. The hours are from 9:30 to 11 a.m., 1 to 3 p.m., and 6 to 8 p.m.
Children must be five years old on or before September 1, 2006, to be eligible to enter kindergarten for the 2006-07 school year. To facilitate registration, parents should bring the child’s official birth certificate (not a hospital certificate) or other government document verifying the date of birth, two proofs of residency (driver’s license, lease, or utility bills), the child’s Social Security number, the family doctor’s name and phone number, and two local emergency numbers.
Children also must have a physical examination before the first day of school. A physical examination is valid so long as it was conducted on or after August 25, 2005.
