Walk-in Registration Set for Wednesday, August 6 at all District 15 Schools
Parents of students who have not registered for the 2003-2004 school year should mark their calendars: walk-in registration at all Community Consolidated District 15 schools is set for Wednesday, August 6, from 7:30 a.m. to 1 p.m. and 4 to 7 p.m.
To register a child, parents must bring their child’s birth certificate and Social Security number, two proofs of residency (driver’s license, gas bill, lease), and family doctor’s name and phone number.
Parents of students who have already been attending District 15 schools should have received enrollment information by mail. Re-registrations which have not yet been completed should be completed and returned immediately.
An immunization clinic will be held on Wednesday, August 6, from 7:30 a.m. to 1 p.m. and from 4 p.m. to 7 p.m. at the Joseph M. Kiszka Educational Service Center, 580 N. First Bank Drive, Palatine.
