Kindergarten Registration Set for April 10

Kindergarten registration for students entering Community Consolidated School District 15 will be held on Tuesday, April 10, at all elementary schools in the district. The hours are from 9-11 a.m., 1-3 p.m., or 6-8 p.m.

Children must be five years of age on or before September 1, 2001, to be eligible to enter kindergarten for the 2001-02 school year. To facilitate registration, parents should bring the child’s official birth certificate (not a hospital certificate) or other government document verifying the date of birth, two proofs of residency (driver’s license, lease, utility bills), the child’s Social Security number, the family doctor’s name and phone number and two local emergency phone numbers.

Children must also have a physical examination before the first day of school. A physical examination is valid as long as it was conducted after September 1, 2000.
