Enrollment Procedures

Parents new to the community may call the Joseph M. Kiszka Educational Service Center at (847) 963-3000 for information about where their child will attend school. Students are usually assigned to their neighborhood school, but in special circumstances, they may be assigned to schools outside their neighborhoods.

Los padres pueden inscribir a su niño/a (debe haber cumplido 5 años de edad en o antes del 1ro de septiembre de 2008) en su escuela primaria más cercana el martes, 8 de abril de 2008, de 9:30–11:00 a.m., de 1–3 p.m., o de 6–8 p.m.

El día de la inscripción deben traer los siguientes documentos:

  • acta de nacimiento u otro documento del gobierno que compruebe la fecha de nacimiento
  • dos documentos que comprueben el domicilio (cuenta de electricidad, cuenta de teléfono, licencia de manejo, contrato de arrendamiento, etc.)
  • el nombre y teléfono de su médico familiar
  • dos números de teléfono en caso de emergencia
  • al niño/a

Kindergarten and new student registration for 2008-09 will be held April 8, 2008. For all other students, enrollment is held in August and throughout the school year at the school your child will attend. For the 2008-09 school year, kindergarten pupils must be five years old on or before September 1, 2008.

You will need the following information to enroll your child in District 15:

  1. A transfer slip from the previous school attended.
  2. An official birth certificate (not a hospital-issued certificate) or other government document verifying the date of birth.
  3. Physician's telephone number and address, if known.
  4. The name and phone number of two reliable local persons to call if you are unavailable in an emergency.
  5. Two proofs of residency (copies of driver's license, lease, or utility bills).
  • My child won't turn five until September 20, but I think she is ready for kindergarten. Does District 15 ever make exceptions to the September 1 cutoff?
  • District 15 follows Illinois School Code requirements that a child be five years of age by September 1 of his/her kindergarten year to enroll. Occasionally, children whose fifth birthdays fall after the cut-off date but before October 31 may be found eligible to register for kindergarten. District 15 has developed specific criteria and procedures to assess children for early entrance to kindergarten. Parents may request an evaluation for the following school year by contacting the principal of their child’s home school any time between March 1 and May 1.   • My five-year old will not turn six until October 10, but he is completing kindergarten in a private school. Will he automatically be eligible to enter first grade?
  • District 15 policy requires children to be six years old by September 1 to enter first grade. Completion of a kindergarten program in a private school does not automatically qualify a younger child to enter first grade. Your child will need to be screened to qualify for early entrance to first grade. For details on screening criteria and procedures, see the flyer, Early Entrance to Kindergarten or First Grade. You may request an evaluation by contacting the principal of your child’s home school any time between March 1 and May 1.

Health Examinations

A health examination/physical examination including immunizations is required by state law. The law applies to students entering an early childhood or preschool program, students entering kindergarten or fifth grade, and all first-time registrants in the state of Illinois. The examiner records the results of the health examination and immunizations on the Certificate of Child Health Examination [SS-722a,b] (English/Spanish) form IL444-4737 (R-01-05). the Health History section of the form shall be completed and signed by the student's parent/legal guardian and verified by the physician. The completed form shall be presented to the school nurse or school secretary prior to the first day of attendance or the student shall be excluded from school. If you object to a health examination due to religious beliefs, you may file a written statement to that effect with the school principal.

Diabetes screening shall be included as a required part of each health examination and the health care provider shall document results of the diabetes risk assessment on the Certificate of Child Health Examination form.

Lead screening using the Lead Risk Questionnaire on the Certificate of Child Health Examination form is a required part of the health examination for children age six years or younger prior to admission to a preschool, an early childhood, or a kindergarten program.

A tuberculosis (TB) skin test is recommended only for children in high-risk groups including children who are immunosupressed due to HIV infection or other conditions, recent immigrants from high prevalence countries, or those exposed to adults in high-risk categories according to CDC guidelines.

Health Information Survey

A Health Information Survey [SS-401] (English Word 32KB/Spanish Word 34KB) form must be completed and turned in to the health office for each child prior to the start of school. This is needed in order to share pertinent health information with school personnel on an as needed basis. This requirement is due to the confidentiality of student health records as defined in HIPAA (Health Insurance Portability and Accountability Act of 1996) and FERPA (Family Educational Rights and Privacy Act). A Health Information Survey form is mailed to homes each summer and is available on the district’s Web site (see above for links).

Dental Examinations

State law requires a dental examination be conducted within one year for all students entering kindergarten, Grade 2, and Grade 6, and for all out-of-state or out-of-country transfer students. For students entering school programs where grade levels are not assigned, examinations shall be completed prior to the date of entering and within one year prior to the age of 5 years and 10 years. If a child in kindergarten, Grade 2, or Grade 6 fails to present proof of dental examination by May 15 of the school year, the school may hold the child’s report card until one of the following occurs: 1) the child presents proof of a completed dental examination; 2) the child presents proof that a dental examination will take place within 60 days after May 15; or 3) the parents obtain a waiver from the Department of Public Health or provide objection to the dental examination on religious grounds. Dental Examination Forms [SS-720] (English/Spanish) are sent home with all students in February each year, or may be obtained at the school office or online. Children in kindergarten, Grades 2 and 6 are required to show proof of a dental exam dated within the previous 18 months of May 15 of the current school year.

Immunizations (en español)

Please be sure that all immunizations and the dates administered are noted on the physical examination form. The following immunizations are required by state law for a child to attend school:

Diphtheria, Tetanus, Pertussis (DTP/DTaP)

Pre-Kindergarten:

  • Has received four doses of DTP/DtaP. The first three doses in the series must have been received no less than four weeks apart. The interval between the third and fourth dose must be at least six months.

Kindergarten or First Grade:

  • Has received four or more doses of DTP/DTaP with the last dose being a booster and having been received on or after the fourth birthday (the first three doses in the series must have been received no less than four weeks apart). The interval between the third and fourth or final dose must be at least six months.

All other grade levels:

  • Has received three or more doses of DTP/DTaP with the last dose being a booster and having been received on or after the fourth birthday (the first two doses in the series must have been received no less than four weeks apart). The interval between the second and third or final dose must be at least six months.
  • If ten years have elapsed since the last booster, an additional TD booster is required.

Polio (IPV/OPV)

Grades K-12:

  • Has received four or more doses of any combination of IPV and OPV, or
  • Has received three or more doses of all-IPV or all-OPV at intervals of no less than four weeks apart, with the last dose having been received on or after the fourth birthday.

In childcare program below the Kindergarten level, 24–59 months of age (includes nursery schools, pre-school programs, early childhood programs, and Head Start):

  • Has received three or more doses of IVP/OVP administered at intervals no less than four weeks apart.

Haemophilus influenzae type b (Hib)

Grades K–12:

  • Hib vaccine is not required for children five years of age or older

In childcare program below the Kindergarten level, 24–59 months of age (includes nursery schools, pre-school programs, early childhood programs, and Head Start):

  • Has received the primary series of Hib vaccine according to the Hib vaccination schedule, or
  • Has received a single dose of Hib vaccine between 15–59 months of age.

Hepatitis B (HB)

Entering 5th Grade for the first time after July, 1997
(Pre-K and 5th through 12th grades only):

Child (age two or older) in childcare program below the Kindergarten level (includes nursery schools, pre-school programs, early childhood programs, and Head Start)

  • Has received three doses of hepatitis B vaccine at the appropriate intervals, or
  • Has laboratory evidence of prior or current hepatitis B infection.

Note: The first two doses must have been received no less than four weeks (28 days) apart, and the interval between the second and third dose must be two months (56 days). For students entering for the first time on or after July 1, 2002, the interval between the first and third dose must be at least four months (112 days). The third dose must have been administered on or after six months of age.

Varicella (Chicken Pox)

Entering Kindergarten for the first time, on or after July 1, 2002 (applies to K through Grade 5) for 2007-08 school year:

Child (age two or older) entering, for the first time on or after July 1, 2002, childcare program below the Kindergarten level (includes nursery schools, pre-school programs, early childhood programs, and Head Start):

  • Has received one dose of varicella vaccine on or after the first birthday, or
  • Had physician diagnosed varicella disease, or
  • Has a statement from a health care provider (including a school health professional or health official) verifying that a parent’s or legal guardian’s description of varicella disease history is indicative of past infection, or
  • Has laboratory evidence of immunity to varicella.

Measles (Rubeola)

Entering at any grade level (K-12):

  • Has received two doses of measles vaccine, with the first dose on or after the first birthday and the second dose no less than four weeks after the first dose, or
  • Has physician diagnosed measles disease or laboratory evidence of measles immunity (A diagnosis of measles disease made by a physician on or after July 1, 2002 must be confirmed by laboratory evidence).

Child (age two or older) entering, for the first time on or after July 1, 2002, childcare program below the Kindergarten level (includes nursery schools, pre-school programs, early childhood programs, and Head Start):

  • Has received one dose of measles vaccine, with the first dose on or after the first birthday, or
  • Has laboratory evidence of measles immunity (A diagnosis of measles disease made by a physician on or after July 1, 2002 must be confirmed by laboratory evidence).

Measles (Rubella, 3 Day)

Entering at any grade level (Pre-kindergarten—12):

  • Has received one dose of rubella vaccine on or after the first birthday, or
  • Has laboratory evidence of rubella immunity.

Mumps

Entering at any grade level (Pre-kindergarten—12):

  • Has received one dose of mumps vaccine on or after the first birthday, or
  • Has physician diagnosed mumps disease, or
  • Has laboratory evidence of mumps immunity.

Change of Address, Telephone Number or Name

In case of emergency, it is very important that the school has the most current address, home, work, and cell phone numbers.

Please notify the school promptly of any change of address, home, work, or cell phone numbers, name, emergency information, or any otherinformation that might be pertinent to school records. The notification should be in writing.

Transfers

Please notify the school when you are leaving the community or moving to another location within the district. The following information is requested from you before you move:

  1. Date of move
  2. New address
  3. New school the child will attend (if known)

Transfers will be ready at the school office on your child's last day of attendance. Prorated book rental refunds will be mailed to the new address.
