Medication Administration Forms and Policy

Effective 5-31-06, Public Act 094-0792 and the Illinois School Code 105 ILCS 5/22-30 were expanded to include both self possession and administration of asthma medication and/or an epinephrine auto-injector for a person at risk of anaphylaxis.

Public Act 094-0792 and the Illinois School Code 105 ILCS 5/22-30 allow a student to carry and administer his/her asthma medication and/or an epinephrine auto-injector at their discretion. It also requires that the school district inform the parent(s)/guardian(s) that neither the district, nor its employees and agents, are to incur liability, except for willful and wanton conduct, as a result of any injury arising from the self-administration of medication by the student. It also states that the parent(s)/guardian(s) to indemnify and hold harmless the school district and its employees and agents against any claims, except a claim based on willful and wanton conduct, arising out of the self-administration of medication by the student.

Due to the expanded policy changes, a new medication form needs to be completed. All students authorized to carry and self-administer their asthma medication and/or epinephrine auto-injector must have both sides of the attached medication form completed. Your child will need to meet with the registered nurse at school to complete the student’s section. Please return the completed medication administration form to the registered nurse in the health office at your child’s school.

Please provide an additional dose of the medication to be kept at school in the event your child forgets or loses his/her medication.
